Yearly reviews of contracts that support deafblind individuals are required to highlight any change in needs or potential change in needs, looking at risk; including health and safety factors.
FACS is able to work within the Social Care Framework to provide these reviews. Additionally specialist reviews can be provided in any situation where circumstances and needs change, for example, placement of deafblind individuals in care services.
Care Planning can offer an individual a planned and proactive approach to the specific needs of the deafblind person, that are embedded in the personalisation of care and services, promoting independence and reducing inequalities.
This supports the concept of working with a collaborative approach between different agencies to provide best practice and service to deafblind individuals.
Communication Support Plans
Individual communication support plans, as a result of a Specialist Assessment, or review of needs can be written in various situations where the needs change or information needs to be shared between carers, family, staff and key workers.
